What Is a Bio Data Form in the Philippines?

A Bio Data form is a personal and professional information sheet used for job applications. It is more structured than a resume because it includes fixed fields like age, civil status, religion, height, weight, emergency contact, government ID numbers, educational background, employment record, and character references.

It is commonly requested for local hiring, walk-in applications, agency submissions, retail jobs, food service roles, security agencies, factory work, housekeeping, driving, construction, and other roles where employers use a standard applicant form.

For BPO, remote, tech, corporate, nurse, accountant, and multinational jobs, a modern resume is often better. You can compare formats in the resume templates gallery.

What to Include in a Philippine Bio Data Form

1. Position Desired and Date

Write the exact role you are applying for. Avoid vague labels like "any position" unless the employer specifically told you to write it.

2. Personal Information

This section usually includes your full name, nickname, address, contact number, email, place and date of birth, age, gender, civil status, citizenship, religion, height, weight, and languages spoken.

Use a professional email address and a phone number you can answer. If you do not want to share sensitive fields like religion or weight, leave them blank unless the employer requires them for a valid job-related reason.

3. Emergency Contact

Write the name, relationship, address, and phone number of someone the employer can contact during urgent situations. Choose someone who knows you well and can answer calls reliably.

4. Educational Background

List your school, address, course or degree, and years attended. If you are a fresh graduate, include awards, honors, OJT, and skills. For more examples, read the fresh graduate biodata sample Philippines guide.

5. Employment Record

Include company name, company address, your position, and dates worked. If this is your first job, write your OJT, internship, volunteer experience, or leave the table blank and highlight your education and skills.

6. Character References

Choose references who can confirm your work ethic and character. Teachers, supervisors, team leaders, pastors, barangay officials, or previous employers can work. Ask permission before listing them.

7. Government IDs and Certificates

Common Bio Data forms include fields for SSS, TIN, PhilHealth, NBI, passport, residence certificate, and similar IDs. Only fill in numbers you already have. Do not invent a number or use someone else's ID.

Bio Data Form vs Resume: Which One Should You Use?

Use a Bio Data form when the company asks for a traditional biodata, when applying in person, or when the role is in local retail, service, agency, security, factory, construction, or similar industries.

Use a resume when applying online to BPO companies, corporate employers, multinational companies, remote jobs, tech jobs, nursing roles, accounting roles, or positions that use applicant tracking systems. The Biodata Format Philippines 2026 guide explains the broader differences, while the biodata sample for job application Philippines guide gives a full sample.
