1. Government Statutory IDs & Tax Numbers
Social Security System membership number or E-1 registration form for statutory benefit contributions.
PhilHealth PIN & updated MDR sheet from local express office or online portal.
12-digit Pag-IBIG Member Identification number and Member Data Form (MDF).
Tax Identification Number. Submitted via employer using BIR Form 1902 (First-time jobseekers) or 1904.
2. Background & Legal Clearances
Valid NBI Clearance issued within the last 12 months for local employment.
Official police clearance from the National Police Clearance System (NPCS).
Clearance issued by your local Barangay Hall confirming clean record and residency.
3. Vital Records & Medical Exam
Official Birth Certificate issued by the Philippine Statistics Authority (PSA) on SECPA paper.
Required for married individuals for tax exemption and dependent health benefit registration.
Complete medical laboratory results: Chest X-ray, Complete Blood Count (CBC), Urinalysis, Stool Exam, Drug Test.
Recent ID photos (White background, collared shirt, with name tag).
